I could see a few scenarios where the US trades nukes with someone, most likely Russia. The main scenario in my mind has China behind it since they have pretty much bought our government. They use the US to eliminate their geographically closest rival, Russia, and the win-win is that the US gets creamed in the process, leaving China in the same position as the US was in September 1945. Whether they understand the impact of fallout and its lack of adherence to national boundaries is another matter. Given their deep and warm regard for their own people, the people of other nations/races and the environment in general I'd tend to think they wouldn't give a damn.

Other scenarios based on time tested motivations by any country playing the game of power follow the idea espoused by Regan, "of the four wars in my lifetime, none came about because the U.S. was too strong".
