I've never seen it but the LGS near here, run by a guy I've known well for over 20 years, has had numerous folks walk in with loaded guns that they "didn't know it was loaded".
Ditto re: the last part..
I have had a couple customers come in with loaded rounds BUT I was informed of it beforehand due to malfunctions that prevented its removal so was fully prepared..
