documentary about the titan II mishap in Arkansas in the 1980's saw this on PBS last week, or two weeks ago, very good and sobering http://www.commandandcontrolfilm.com/A chilling nightmare plays out at a Titan II missile complex in Arkansas in September, 1980. A worker accidentally drops a socket, puncturing the fuel tank of an intercontinental ballistic missile carrying the most powerful nuclear warhead in our arsenal, an incident which ignites a series of feverish efforts to avoid a deadly disaster. Directed by Robert Kenner (FOOD, INC.) and based on the critically acclaimed book by Eric Schlosser (FAST FOOD NATION), COMMAND AND CONTROL is a minute-by-minute account of this long-hidden story. Putting a camera where there was no camera that night, Kenner brings this nonfiction thriller to life with stunning original footage shot in a decommissioned Titan II missile silo. Eyewitness accounts — from the man who dropped the socket, to the man who designed the warhead, to the Secretary of Defense— chronicle nine hours of terror that prevented an explosion 600 times more powerful than Hiroshima.
...Actually Sycamore, you are sort of right....

Color me skeptical. Not about the fact that it was a dangerous incident, but about the claim that it could have caused a thermonuclear explosion.
Warheads are built with some sort of goo/foam that is released in an accident. It prevents the fissionable material from making intimate contact.
Be not weary in well doing.

the trailer is just a trailer, but the movie was good, I think you can stream it at http://www.pbs.org/wgbh/americanexperience/films/command-and-control/player/90 minute show. there are a number of built in fail safes, apparently, but you know Murphys law. In this case, according to the movie, there was no electricity to the warhead, so it did not initiate. Sycamore

But that's all.
