don't know what caused it but my Navy issue 1911 target pistol did the same thing. The small arms marksman instructor wasn't happy with the pistol. said the people building them (Navy) didn't know what they were doing. Fortunately I was only practicing. It would have been ugly during a match. or uglier.

that was 1966 or 67. Nothing but cameras back then and none were around when it happened. Or if you mean the work on the 1911's I don't know of one.
