Last week a friend shot the choke out of his OU Mossberg. He found the tube and it measured .004" undersize. The other chokes screwed in tight, but he hasn't fired it since. Has anyone else had this problem?

are you talking about the outside dia or the inside dia? Have seen several that the inside were not as stated, but I am guessing you are talking about the outside.
I may not be smart but I can lift heavy objects
I have a shotgun so I have no need for a 30-06.....

It was a modified choke and they were just outside skeet shooting.

You need to increase your lead when shooting chokes.

My force of habit is to finger check the chokes after every station while shooting trap/skeet/sporting clays. Fairly common for them to loosen up whilst shooting.

My guess is they teal is right on, chokes get loose and ya need to keep an eye on them, extended tubes are easy for just that reason. If the threads are OK just keep shooting and try to not let it happen again.
File it under chit happens and dont let it bother you

I get to clean the 4-H shotguns every Wednesday in summer. Two are 1100's in 20 ga., one made right after the other. On one the choke tube will loosen, sometime up to almost 3/4 turn, the other tightens if anything. Go figure!
