It is a staff set in a receptacle at the transom of the vessel. (Or on the flag mast, opposite the signal halyard, as a courtesy flag for cleared foreign vessels in operating in American waters)
By design the transom receptacle gives the staff a sternward tilt which sets the stars to lead.
The flag should not be so large that the flag reaches the water when the vessel is at rest.
When the vessel is at rest the ensign should be struck with the playing of "colors".
When underway the stern light meets the requirements of "lighting the flag" so the ensign may be carried after sunset.
