I understand that puppies under 4months old cannot be imported from Canada unless USDA approves a quarantine. Has anyone been successful in this quarantine approval process?

Not from Canada, but from the EU. The quarantine takes place at your home. It's a paperwork formality, at least it was in my case. Have a USDA certified vet do a checkup and say you'll keep the dog on your property.

At least, that was my experience.
