I had a scaffold get away from me - went off the stage in an auditorium/basketball court.
Thank God I was young, strong, and agile. It took a while for my apprentice to re-assemble the works, on stage, so i could get out of the rafters - 24 feet above stage deck, IIRC.

The lanyard on safety harnesses is in back. Once you get hung up on one, you have to wait for help. I've never fallen when wearing one but I'm told that it'll really jar you.
