Remember we are talking career bad guys, not guys who played one bad role like Anthony Hopkins.
I think Anthony has played quite a few 'bad guy' roles over the course of his long career other than Hannibal Lecter. I'd have to do some research into it, but one that comes into mind off the top of my head is the recent release "Fracture", where he played a tycoon that thought up a diabolical plot to murder his adulterous wife. He's excellent in these types of roles.
Walken certainly has to be in the running. Remember him in "True Romance"? The scene between him and Dennis Hopper is the stuff of cinematic legend.
Dern is another good nominee. Even when he's not being outright villainous, think "The Great Gatsby", his character and performance usually scream "DOUCHE BAG".
I'd add Gary Busey to the mix. Not only does he actually have the psychotic element that makes a great villain, he's a good enough actor to make his roles believable.

Check out a recent French horror film called L'Interieur (Inside). I'm fairly confident that this would easily replace "SE7EN" as the most disturbing film on your list. All I'll say is that you are the person you were BEFORE you see this film and then you are the person you become AFTER you watch this film. Has to be the most DEPRAVED movie I have ever seen.

There are similarities in terms of the complete and utter depravity but I consider L'Interieur to be much more shocking. There has been a recent rash of horror films from France that seem to go way beyond the accepted norms of violence and violation of taboo topics. Not sure why. A couple of others in the same mold are "Martyrs" and "Haute Tension". These are definitely not your Jerry Lewis-type flicks.
The toothy, weasel-like varmint played by Bruce Dern shoots John Wayne in the back in the movie Cowboys.
I saw an interview of Bruce Dern years after the Cowboys was filmed. He said that the day before the movie was to be released he and John Wayne were at a Hollywood function and the Duke was laughing and leering at him. He went to Wayne and asked what his problem was, to which the Duke replied "Tomorrow you are gonna be the most hated man in the world. You will always be known and the a$$ that shot John Wayne in the back".
Bruce said that Wayne was right, and that everywhere he went people always reminded him of that.
I know he's probably not in the "career" category yet, but this guys character is sure scary. Anton Chigurh played by Javier Bardem in "No Country for Old Men"
