I know you are a physician, and I am not assuming I have more knowledge than you on correcting a messed up Na level, but, in the ICU setting, with frequent checks of serum Na levels off an art-line or central line, 3% saline replacement is no more dangerous than most IV infusions.

The key is "ICU setting" and "frequent Na checks."

Then again, if we're replacing with 3% in the neuro ICU, you are already flirting with death......

Both times, my Na was checked every 12 hours. Both times, my IV was a Na rich one........specifically labelled as such versus a normal one...........but I don't know how much more. Plus they gave me a Potassium bag the second time. I like bananas better.............

If you are getting 3% and your Na is checked every 12 hours, they're doing it wrong. I work in the ICU and don't mess with anything other than normal saline (0.9%) or 3%.

In the ICU I work at, Q4 hours is pretty normal, and more frequent not really abnormal, depending on circumstances. We usually have a line in the patient, so are not sticking them to draw blood.

Doc was not kidding or exaggerating when he stated that giving replacement too fast is a highway to disaster. We have a specific protocol we run every few/4 hours that titrates the 3% drip to the desired results over a specific amount of time.

I was pretty irate at only having blood pulled twice a 24-hr day. It should have been more frequently. Please poke me now..........

The IV bladder showed a higher level of Na...........it was labelled as such.......not sure what it was but it was higher than a 'regular' IV. Trust me.

Sons-of-guns, they get your good insurance card and then they run with it. No kidding.

The Boss had a problem with electrolyte imbalance. After going to the hospital for a couple of IVs my grandson told her to get some Pedialyte in the baby section. He said that when cutting weight for wrestling dehydration was a common problem.... The Boss tried drinking a glass every morning and hasn't had a problem since...


I am not ppine, please don't confuse me with him....

goalie... yeah, I know you guys in ICU can and do work with 3%, but that's ICU. More power to you! In most ICU's the nurse:patient ratio is 1:1, and in the ER it can be 1:3 to 1:5. I'm not saying I don't trust my good ER nurses, but there's just too damn many distractions in an ER.
