Not a Doctor and don’t play one on TV .... but .... it looks like you have a blood platelet issue. Simple blood test will tell if your platelets are low. If so, then the next question is why?
I got a big bruise on my arm right now, I don't recall why. I take warfarin and I don't know what all (have to ask the wife). Cutting , hauling ,dragging brush will do it every time.

Is the skin flap toughened somewhat? Very faint red plasma come outta it after squeezing for flow outta it. Dry loose secured taped outer edge gauze bandage for air into it.

I asked my dermatologist as I have the same conditions on the back of my hands. I do take two baby aspirin every day... He said over 50 and the aspirin. I know the OP said that he didn't take blood thinners, but the over 50 must be a big factor.

uncle had a heart transplant meds kept him in depression. he got tied of it one day and capped himself through the heart. i knew it was coming he hinted to me more than once, don't blame him for his choice.
