That is only the front side. There is also some verbiage on the back side.
If one is semi capable at manipulating a computer PDF file, it is quite easy to print one at home with the proper card stock.
There are a few sites on the webs that give out detailed instructions.

If it works for fake DL's and fake SS cards for "undocumented aliens" I don't know why it wouldn't work for this....
Although some couple from Florida did just get nailed in Hawaii for a fake card. And unlike illegals with fake DL's and SS cards, I'm sure they'll drop the hammer on them.

There are plenty of official sites that have had images or PDFs of the full CDC card available for months. Here is Florida's the shows both sides.The probably illegal part of making one of these is the use of the CDC seal. It would be interesting to see it tested in court though, because the seal is on blank cards so no one would actually be "affixing" the seal, just filling out the card.
