Orbital blowout fractures are more typically associated with MVA's, fall from a significant height, etc. In an altercation, it implies some kind of bludgeon/club being used, or "brass knuckles", etc. It indicates a much higher level of energy and, one could argue, a deliberate attempt on the part of the assailant to inflict grave bodily harm.
