I have arthritis in my shoulders, and gave up shooting the big stuff. I loved the way my SBE2 3 1/2 patterned and killed turkeys, but hated the way it kicked. I shoot 20's now, although I'll sometimes sneak in a hunt with my first dedicated turkey gun, a Winchester 1300 3 inch gun.
The 20's will kill just as good as 12 will, without the pounding of the shoulder.
