I'll bet they turn around a BNIB one and repair/clean the one you sent in, then resell it on the SampleList.

The three specs are likely grease used in the assembly process. The grease may have been applied a bit too liberally and it got thrown onto an internal lens surface under recoil. There was a time when this was extremely common phenomenon across the industry.
