Probably been all sorts of testing done in Remington labs that failed to have it happen. Most likely when it did happen it was because some idiot had his finger on the trigger or failed to properly baby it in terms of maintenance. 
Not a real member - just an ordinary guy who appreciates being able to hang around and say something once in awhile.
