Nope. Bad idea to have sex within about three days of a psa draw. It stimulates the prostate and give a false high reading. Same thing with riding a bicycle.

That's about the only chance you have to catch prostate cancer early. I fired my family Doc last July for being stupid and went to see another family Doc in same clinic for my yearly check up. I told him the main reason I was there was to get my PSA level checked because I have a family history of prostate cancer. He told me that after age 70 you will only get the slow growing type of cancer so you don't need to worry about it. He refused to order a PSA test. He ordered some lab work but this was the first time I ever had a yearly exam where I didn't pee in a cup, have a PSA test or have a lipid panel done. The nurse called me to come in for some more lab work and I told to tell him I'm never coming back. I haven't had a digital prostate exam since 2012 at this same clinic which is very important if you have a family of prostate. I PAID $23 TO HAVE A PSA done at a local lab and my level is less than 1 still.

These are some my personal experiences with the people I've known who got prostate cancer:
1) My Dad had prostate cancer when he was in his mid 60's (never had a PSA test) but I didn't know that until after he died of a stroke at age 70 and my aunt told me.
2) My Dad's Dad died of undiagnosed prostate cancer when he was 89. He was very healthy otherwise but didn't survive the prostate surgery.
3) A good friend in his early 50's found out that he had prostate cancer when he started having problems peeing. It was cured with radiation treatments. That was 30 years ago.
4) A good friend who just turned 70 starting having problems peeing and turned out that his PSA level was elevated (normal the previous year). An MRI showed he had prostate cancer and he's having surgery next month.
5) A good friend who just turned 73 gets a PSA test and digital exam every 6 months because his Father died of PSA cancer at age 89. His first PSA test year was normal and the 2nd test was very high. Am MRI showed he had prostate cancer and he's also having surgery next month.
6) My SIL's Dad ( who never had a PSA test) started having problems peeing in his late 60's and waited until he couldn't pee before he went to his Doc for help. He had prostate cancer that had spread. He had surgery, chemotherapy and radiation treatments. The chemo damaged his heart muscle and he died of a heart attack at age 74.
7) A good friend found out when he got a colonoscopy at age 70 that he had prostate cancer. Up to that point his family Doc point had never ordered a PSA test. He had prostate surgery and 50 or so radiation treatments and appears to be ok He 73 now and his Urologist orders a PSA and gives him a digital prostate exam twice a year.

I have an appointment with a Urologist next week that worked at MD Anderson for several years. He orders PSA tests and does digital exams every year (sometimes two). I'm going to start seeing specialists as much as possible. Any Doc that tells a 70 year old man that he doesn't need a PSA test needs to be kicked in the nuts. He'll change his tune when his Dad and himself are 70.

If you’re concerned about what your PSA should be relative to your age go to Memorial Sloan Kettering (MSKCC) site and it breaks it down by decades of life.

Hmmm wonder if the morbidity rates for rarelys would be higher, or lower than bicycle riders.?

PSA levels are virtually worthless, they are like a weather forecast. 50% chance of rain. That means the chance is less than a flip of coin.

The levels for lab values have been raised in the last 5 or so years.

Thoughts on biopsies?

Absolutely not. waste of time.

There is no question that each and every male has prostrate in some state of abnormal growth. At the same time the bladder will be showing abnormal cell "flaking" which depending on which medical facility you go to will call it a "cancer" or not, then make recommendations.

So, for a patient with an enlarged prostate and elevated PSA how would you differentuate between a slow-growing cancer and one that's mire likely to metastasize?

oz of prevention is worth a lb of cure.

elevated PSA doesn't mean anything , you have more than 1 slow growing cancer in your body already,

I'll re-state the question: For a man with all the symptoms of prostate cancer, how would you go about making sure he doesn't have an aggressive form that's likely to metastasize? Use an ounce of prevention?

When my mother-in-law was raising money for breast cancer awareness wanting everyone to wear pink. I asked her what the color of prostate cancer would be; brown or yellow. She said she would look into it. Never did find out.

Yep, they sure are. Nothing like having a stapler shoved up there and stapling your prostate to your colon. I recommend for everyone who is looking for a new and unique experience.

I had 100% malignancy in one lobe and 60% in the other. A great surgeon at the UC Davis Cancer Center did daVinci surgery, and I was back working in three weeks. Quit needing pads at six months, and most "function" came back in twelve months.

Serendipity lead to finding my cancer, as did my old friend and former schoolmate DR who had a hunch. Thirteen years now, and I'm cancer free so far.

GET CHECKED!!

Much thought provoking info here.

Luv2safari, you and Hubert have had your trials with this.

Yes, but with all due respect the worst case scenarios always get the most press.

For each bad one here there are 1000s with better out comes, even using the traditional medical machinery

and 10000 even better outcomes by using preventive medicine. But then you never hear about them.

There are several different strains of prostate cancer. The only way to find out which one is a biopsy. Not pleasant but not that bad, there are worse things. Some strains are more aggressive than others, and the very aggressive ones can metastasize rapidly. You really want to catch it as soon as possible. Also to say that PSA doesn't matter isn't true, it does. Prostate Specific Antigen is produced to fight something in there. At the very least an enlarged prostate and elevated PSA is a sign that things need to be checked out. So go to the best clinic you can find and have tests done. Nuclear x-ray will show if it's metastasized and where.. I've been through this. Good luck to you.

Absolutely correct on every point! I had an aggressive form that my alert doctor found just in time. The surgeon in Sacramento got the test results and history from me in an email about 12:00 on a Sunday night and emailed me Monday morning about 6:00 and asked me to come down that day. A couple of days later (all due to insurance verification and approvals) I was being slit six places for the robotic surgery. Only 4 months earlier my PSA had tested only slightly high.

Jumping on cancer is never too much of a rush, even prostate cancer.
