No farting......pretty sore.

If the pain is constant, I had the same thing a few years ago, constant pain, felt like my guts tied up, puking, dry puking, felt like I needed to chit but couldn't... went to er and they said kidney stone, I thought no it's in my guts not my back... but they were right, kidney stone.
