If you had a 375 H&H that had lots of sentimental value but the bore and chamber were ruined would it be possible to sleeve the barrel with turned down .375 barrel and rechamber leaving the external part of the barrel as original?

Maybe, if the tolerances are tight. Warm up the barrel after drilling and cool the sleeve, insert and hope they join up tight. Some old gunsmiths used solder to join barrels. Would that be better than epoxy ?
