How difficult would it be to rechamber a commonly found .223 Contender Carbine barrel from .223 to (the rimmed) 219 Donaldson-Wasp?

Aside from a 30-30-sized extractor, would anything else be needed?

Could it be done by hand, or would it require a lathe?

You want to have the rim counterbore cut on a lathe. I had a rim counterbore cut on a 300 BLK single shot barrel by a "hobby gunsmith" that was terribly out of round and not the same depth. He used a drill press and a "special" reamer. Never again!

Hand reaming the chamber may be possible but only if the reamer pilot reaches into the bore and you have a way of ensuring there's no tail runout. The only hand rechambering I've done were 357 Mag to 357 Max and 44 Mag to 445 Supermag. In both of them the reamer was fully supported by the original chamber, the pilot reached into the bore, and the rims were already cut.

Eabco sells barrels already chambered for the Wasp, good shooters and less trouble to get.
