I knew a guy from the south and he said it in positive context too. If someone was sick etc, he would say "oh bless her heart." It was really quite gay and annoying.

It all depends upon the inflection. It can be said in a positive way, or with condescension as in "bless his heart, he's just stupid", with the "he's just stupid" implied and left unsaid.
If it's said by a guy it's usually meant to be condescending, because it does sound gay and annoying. Women will say it and actually mean it, from them it just sounds sweet.
