The 40 is a hot little stubby bastard.

I don't have all the details yet,but the other three I seen have did the same damage. Two were reloads, the other factory. This happened on the forth shot

Interesting that is a 4th generation Glock.

In times of ammo shortages, there has been some chitty ammo on the market. I was shooting some .45 ACP ball loads, and the Les Baer choked on them. I realized the bullets were setting back in feeding.

I've read if the bullet sets back in feeding on a .40, pressure can rise dramatically. With the Grock having a somewhat sloppy chamber, high pressure = blown case = broken plastic frame.
Blow a case head in a steel gun, they frequently survive.

I blew a case head in my G20 10mm and the frame was fine. I only lost the extractor and my hand was a little bruised. Don't know what caused the hand injury. I was shooting some unknown round count used brass. I only use new now.

I have long used only new Starline brass for my Glock 20 10mm loads with 180 grain XTP's, reload once with a plated 180 gr HP and toss the brass.

Shooting pals 27 has blown case heads twice in my presence and at least one other time. Mag blew out and stung his hand no damage to mag or anything else. His reloads (not hot) but had smiley before reloading. He quit reusing brass.

All Glocks have an inherent design defect...they will fire out of battery. And when they do the slide comes back faster than the bullet can get out the barrel and the pressure will seek the path of least resistance...the unsupported part of the case web.
I have been present when two Glock 19s went KaBoom...one was with my reloads which were being fired in a coworkers gun and the other was Winchester 115 white box. My personal 9mm/34 had a KaBoom with a friends reloads. None of these resulted in any damage to the gun.
A friend's .45/20 wasn't so lucky in that it went to pieces while I was looking at the gun being fired. This was with 230 FMJ reloads. Glock rebuilt his gun...no charge. Another students 23 blew with factory .40s right in front of me. As he lived in Massachusetts where Glocks are banned from sale Glock has his gun but because the frame had to be replaced they can't return the gun to him as technically it is a different gun. That was in late 2012 and last I heard he still didn't have the gun back...and that was his only gun. He borrowed my 19 for followup classes.
One of my co-instructors had his 23/.40 blow on a reload. Glock put it all back together no questions asked...
It is why I stay with 1911s...they may not be Perfect(ion), but they also don't claim to be....

Rule number one about dog training is, you've got to be smarter than the dog. Anybody can train a lab, no so many can handle Chessies.

No, they usually don't fare well. The gas goes down the magazine and expands the magazine and bulges the magazine well, pretty much wrecking the frame.

Would have to disagree Kevin.... Have personally experienced a number of blowouts with my Supers and never did anything but crack the grips. Never even lost a magazine.
Friend with a Colt 1911 GM .45 was competing and had what he thought was a misfire that was actually a squib half hay down the barrel. He did the tap-rack and pulled the trigger and it blew...
The gunsmith who built most of my Supers in Texas put a new barrel in, straightened the dust cover and put on a new set of grips...and my friend had the gun up and running for the next months shoot.
I have seen some steel framed guns wrecked by a double charge, but not by a simple blowout of a standard pressure round like happens with a Glock...
