What on earth that yellow mass on the side of the head might be I dunno. Dipterans (flies) escape the pupal case by means of a sort of air-bag looking device on the front of the head. Basically the head capsule opens and a bag pops out, breaking the pupal case. Maybe in that one the airbag didn't deflate properly.
