I kicked up another thread that mentioned it was a Kershaw with a gut hook, and he was using the hook to cut the twine. I only found one folding model with a hook, and it looks like the user's fingers would be wrapped around the lockback release bar if holding the knife upside down.
I grew up with slip joints, and still treat lockbacks with the same respect. For me it's the same as never using/trusting the safety on a firearm.
Dead on- he was apparently using it upside down, with pressure on the back of the blades while applying pressure to the release... What a maroon... not especially for the mis-use, but for the suit.
Ditto on the safety thing also.
