On the 2 point conversion, the guy that called the play and the lineman that eventually caught the pass are both supposed to know that the lineman needs to report as an eligible receiver.
They both failed.

In the NFL, the numbers 50-79, and 90-99 are deemed ineligible numbers. However, the NFL permits someone wearing these numbers to "report as eligible" to the referee before they line up. Once they do so, and the referee has announced to everyone that "X" is eligible, that player must still line up in one of the eligible positions listed in point one (behind the LOS, or as an END on the LOS). If they were to line up as an "interior lineman," they would no longer be eligible.

We noticed there were two flags thrown on that same play. Was there another penalty called on that play as well?
