170k on my '05 2500. 5.9 6spd manual. I used it in my Heavy equip business for several years, and since as my primary daily driver. It's pulled quite a bit and done quite a few miles on ranch roads also.

Only repairs I've done other than tires/brakes/batteries/belts:

I replaced the injector pump ($1k)
One injector line ($100??)
about 160k had ball joints etc. rebuilt in front end ($1500)
A/C blower motor ($150)
Hood shocks ($100)

I don't consider those repairs excessive at all for the good service I've gotten out of this one, It's been a great pickup. I don't think I'll ever sell it.

On a 12 yr old 200k+ mile pickup you will have to fix little things. Especially if it's bounced around dirt roads. For instance one of my rear doors just quit opening last week. Seems linkage between the handle and latch has come undone, I'll have to pull inside panel to fix it one day.

If the the auto trans is original I'd ask a dodge mechanic about typical service life on those, especially if it was pulling frequently.
