In complicated cases like this...horribad disease, multiple consults, transport issues...there should be an active duty case manager assigned. (As well as a tumor board made up of experts to help provide best care tailored to the case).

That person - the active duty case manager - helps the primary care manager (his doc) coordinate everything. They are the one's to ask your facility transport/treatment questions too...they are usually an RN.
