Another reason for that "tugging up in my guts" and for the perception that "local" doesn't work is that your friendly 10th cranial nerve -- the vagus -- heads south from your brain base bypassing the spinal nerves and ends up permeating almost all of your viscera; that is, all your innards.

You can block the tissues all around those little tubes with local but retraction of them (lifting them up) with an instrument which is necessary to cut them will transmit that little action upstream to your brain and that cannot be blocked. Thus cramping, uncomfortable pressure, etc ensues. It's one reason a spinal or epidural anesthetic in itself is not sufficient for any bowel surgery even an appendectomy though you may be numb as a stump from nipples down.
