The only trick is to carefully cut the top off the packet, leaving the hand warmer inside. The packaging keeps the hand warmer from getting wet, while providing access to enough air for it to work.
ON wilderness trips, and particularly fly ins, where space and weight can be tight, you can always find room for a few of these.
