My guess, like every other form of insurance, it's sky high because of scum sucking lawyers/litigation costs.
I expect that's part of it... but the medical econ papers I've read state pretty clearly that more than 80 cents of every health care dollar goes to administration and regulation. The actual dollars paid to the people who actually care for patients is less than 10% of the total.
