The intent of the ACA was to render open market insurance companies insolvent. It was the first step needed to move to single payer. Americans were not going to do it voluntary so an intermediary step was created - the ACA. As co-author Jonathan Gruber said, paraphrased - we will rely on the stupidity of the American public to sell this. The poison pill put in the plan was the co-mingling of about 10M high risk citizens whose medical bills sent everyone else sky high. We have a shot at returning back to pre-ACA status or have single payer. Medical attention will be on par with how the VA delivers it.
