While we can't cover every scenario, beleive it or not, most Medical practices these days fit your definition of "high volume". They see patients in many settings every 20 minutes. Problem is, to do you job thoroughly takes 30 minutes, so by 10:30 am every day they are 45 minutes behind.

In a clinic situation, there is no incientive to bust your ass. If you are going to be there for 10 hours and are paid salary, why bust your ass? Saw this in military dentistry. They are not breaking a sweat most days, and sure don't like the transition to private practice in many cases. Work too hard! smile
