One thing that would change the calculus immediately for the better, and would put the money where it should be, is to allow people to practice medicine without a medical license. Pay out of pocket at negotiated rates to a person you trust.
Medical doctors are rarely needed for the vast majority of illnesses for which people seek treatment. An RN could treat most all of them just fine.
