You don't seem to understand what free will is. Free will is a choice by a sentient mind. A random brake failure has a cause and effect, but is still not due to free will.

brake failure alone don't necessarily lead to death. drivers can make different choices in the event of brake failure that can have very different results.
Recently a remote volcano errupted, not an accident, and not a natural calamity as a stand alone event, IT was the choice people made to risk visiting the volcano that resulted in their calamity.
